Most insurance companies require referrals. Please check your plan by calling your insurance company’s Customer Service division. You are responsible for keeping track of your referral, including when it expires. You are responsible for contacting your Primary Care Physician for a referral prior to your visit. Most insurance companies send notifications confirming dates of coverage and the number of visits allowed. Patients can call our office to see how many visits remain or if their referral is nearing the expiration date.
